Metabolism is altered as a result of critical illness. The mechanisms are not entirely understood at this time, but we do know that there are shifts in substrate utilization, catabolism, and insulin resistance. If nutrition support is not initiated in a reasonable timeframe, the metabolic changes coupled with lack of nutrition can result in malnutrition and muscle wasting. In addition, pediatric patients lack significant storage of macronutrients and higher nutritional requirements, as compared to adults, making nutrition support imperative.
